Application forms

Applicants must complete the Co-operative Education Application Form (rtf, 427kb) by either

• Typing in answers and printing off completed form
  or
• Printing off the form and handwriting answers
  Handwritten application forms must be in blue/black ink only.

Supporting documentation

Copies of all supporting documentation must be attached to the application form.
Do not send original documentation as it will not be returned.

Mailing address

All applications must be mailed to the Co-operative Education Officer (address details at right)

Due date

Applications must be postmarked no later than 19 December, 2008 to be accepted

Confirmation

Confirmation of receipt of application will be mailed within 7 working days of receiving the application. If you have not received confirmation within this timeframe please contact Kylie O'Donnell in the Co-operative Education Office.
